Benefit Description

Pays $5000 a month after a six month waiting period following an accident, which causes disability for up to 6 months.
Pays $10,000 a month thereafter for up to 120 months if member remains continually disabled after twelve months.
Includes a $1,000 Accidental Death Benefit
